Possibly borrowed from Sicilian spacchimi, ultimately from Vulgar Latin *expatlum, from Vulgar Latin *expatlum literally meaning "what comes out of the [vaginal] opening", from Vulgar Latin ex + *patlum, from Latin patulus (open, spread out).
