The Starfleet Medal of Honor was a medal for valor presented to deserving Starfleet personnel. It was possible to earn clusters for subsequent awards of the medal.

In 2257, members of the USS Discovery's crew were awarded this medal for their role in ending the Federation-Klingon War. These members included acting captain Commander Saru (the first Kelpien to receive the medal), Commander Michael Burnham, Ensign Sylvia Tilly, Lieutenant Commander Paul Stamets, Doctor Hugh Culber (posthumously), Lieutenant Commander Airiam, Lieutenant junior grade Joann Owosekun, Lieutenant Gen Rhys, Lieutenant junior grade R.A. Bryce, and Lieutenant Keyla Detmer. (DIS: "Will You Take My Hand?")

Captain James T. Kirk had earned this medal by 2267. (TOS: "Court Martial") The medal had such significant meaning for Kirk, that it helped him overcome the influence of the omicron spores that year. (TOS: "This Side of Paradise", "The Tholian Web")

Lieutenant Commander Data had received this medal by 2365. He had multiple clusters with this medal. (TNG: "The Measure Of A Man")

In 2399, after it came to light that Commander Raffaela Musiker's theories about the Zhat Vash being behind the Attack on Mars were correct, she was awarded the Starfleet Medal of Honor "for commitment to truth in the face of great opposition." (PIC: "The Next Generation")

In 2367, while exploring a cavern on Alpha Onias III, Commander William T. Riker was rendered unconscious by gases. While unconscious, neural scanners scanned Riker's brain. The scanners used elements of Riker's reality and constructed a holodeck simulation with those elements interspersed throughout, so that it felt real to him. What Riker wanted, the scanners made possible.

In one of these simulations, on stardate 51692, at the conclusion of the Praetor Campaign, Captain Riker of the USS Titan was awarded the Starfleet Medal of Honor. In the campaign, the Titan detected and prevented a Romulan preemptive strike into the Gamma Hydra sector by the Romulans. In 2383, this award was mentioned in Riker's service record that was on display in his quarters on the Enterprise-D. (TNG: "Future Imperfect", okudagram)

For the remastered episode of "Future Imperfect", the original text of Riker's service record was replaced.
