Kamar's shuttle
Garak, who following he and Nog's capture by the Jem'Hadar, attempted to explain to the Vorta Keevan that his name was "Kamar" and that he had been captured by the USS Centaur a week ago, and was being "held aboard one of their shuttles," but was interrupted by the Vorta, who pointed out that Garak was wearing a Starfleet combadge. (DS9: "Rocks and Shoals")
Though clearly a lie, Garak's reference would have clearly been based on factual information, such that Centaur was a real ship and very likely carried shuttles aboard.
USS Enterprise

In 2269, when an investigating landing party was cut off from the USS Enterprise by an electronic block created by the master computer of the Shore Leave Planet, this shuttle was boarded by an emergency rescue party. This two-man shuttle was "borrowed" from the USS Stargazer by Jean-Luc Picard and Jack Crusher for a secret rendezvous on Argelius IV. En route, they encountered a micrometeoroid shower and were disabled. Picard told the story of this incident often, referring to it as his personal "no-win scenario". (PIC: "No Win Scenario")

USS Voyager
Tuvok and Chakotay's shuttlecraft
This USS Voyager shuttlecraft was piloted by Commander Chakotay and Lieutenant Tuvok on a trade mission with the Ilidarians in 2371. On their return trip to Voyager, the shuttle was attacked by the Komar and was badly damaged, suffering multiple hull fractures. With Chakotay and Tuvok unconscious, it returned to Voyager under autopilot. (VOY: "Cathexis")

Although Chakotay was the ranking officer, this craft was identified onscreen initially as "Tuvok's shuttlecraft" and later "Tuvok and Chakotay's shuttlecraft".
Chakotay's and Paris' shuttle(s)
This shuttlecraft was was taken by an away team led by Chakotay to the surface of the symbiogenetic alien orchid homeworld to collect fresh samples of the native orchid for further study, after the transporters were deemed unsafe, following the creation of Tuvix. Chakotay later returned with a few samples on the shuttle that Kes ran a biochemical analysis on.
A second shuttle trip was taken to the surface by Tom Paris and B'Elanna Torres at 06:00 the following day. Together they brought several flowers from Voyager's airponics bay, that were used along with the medical transporter, in an attempt to reverse the symbiogenesis process that created Tuvix. (VOY: "Tuvix")

This shuttle was piloted by Neelix' as the crew of Voyager provided supplies for the Borg Cooperative. It was joined by a shuttle piloted by Chakotay and B'Elanna Torres.
After Chakotay was coerced by his mind link with the Cooperative to head towards the nearby derelict Borg cube, Voyager was forced to pursue, leaving Janeway to report to Neelix that they would rendezvous with his shuttle later. (VOY: "Unity")

Paris and the others' shuttles
These shuttles, one piloted by Tom Paris, with a second piloted by "the others," joined the Tuvok-piloted Sacajawea with transporting rescue teams down to the Nezu colony planet in 2373 in order to locate Doctor Vatm.
Following the crash of the Sacajawea and the subsequent discovery of Doctor Vatm, Tuvok inquired if he had "seen any other vessels like ours", which Vatm had not. Meanwhile, on Voyager, Chakotay notified "Lieutenant Paris and the others," of Tuvok's shuttle having gone down. Paris proceeded to perform three low altitude survey's, but found no sign of Vatm or the downed shuttle. Janeway relayed that they were monitoring a major dust storm heading their way, and suggested he should "head due north within the hour." (VOY: "Rise")

Paris and Jetal's modified shuttle
This shuttle was a project vehicle worked on by Ahni Jetal and Tom Paris on Deck 11. The project kept them very busy, as they were attempting to modify the shuttle to make in "more maneuverable and more cool." (VOY: "Latent Image")

Chakotay's shuttle
This shuttle was piloted by Chakotay during a survey mission. During the mission, he picked up traces of omicron radiation in the atmosphere of a planet populated by the Kradin and Vori. As the shuttle slowed down it came under attack, Chakotay lost helm control and had to make an emergency transport to the planet's surface.
According to the Vori that he later encountered, Brone, Chakotay's shuttle had strayed into the war between the two species on the planet. Though Chakotay explained that he tried to explain that he had no hostile intentions, his attackers kept shooting. Brone explained that the Kradin maintained air supremacy over their enemy, and would fire on anything that didn't display Kradin colors.
He further explained how one of his defenders had observed the shuttle falling into Grove Yellow. Chakotay, nevertheless, wished to seek out his shuttle, hoping that it was perhaps salvageable, a desire Brone thought to be unlikely. The following morning, Chakotay found a piece of debris from his shuttle, observing the obvious... "it doesn't look like there's much left of my shuttle."
Two days later, USS Voyager finally found the same planet and located what was left of Chakotay's shuttle. Unknown whether Chakotay survived the crash or not, a preliminary analysis of the shuttle wreckage found no evidence of cellular residue, indicating that there was a chance he had survived the crash. (VOY: "Nemesis")

Kim and Ballard's shuttle
This shuttle was piloted by Harry Kim and Lyndsay Ballard to a class M planet in the Vyntadi Expanse to collect some dilithium ore that was previously detected.
After the shuttle landed, they realized it was a trap set by a Hirogen hunting party, so Harry and Ballard headed back, only to have Ballard shot by a neural disruptor when she was ten feet from their shuttle. (VOY: "Ashes to Ashes")

Janeway and Seven's shuttle
This shuttlecraft was modified to specifications given by Captain Kathryn Janeway following the issuance of the Omega Directive.
Tom Paris was initially told to modify a shuttlecraft to withstand extreme thermal stress of at least 12,000 Kelvin. B'Elanna Torres also worked to reinforce the shuttle's hull, with Harry Kim later joining her. After completing the modifications and collection of necessary resources for their mission, Janeway and Seven of Nine were to leave at 06:00 the following morning.
She further instructed Chakotay that the shuttle mission was going to end one of two ways: "Either Seven and I will succeed on our mission, and return within a few days, or your long range sensors will detect a large explosion in subspace. If that occurs, you'll have less than ten seconds to jump to warp and get the hell out of here. Head for the Alpha Quadrant and don't look back, understood?"
After some convincing, however, Chakotay was able to allow the Janeway to allow the crew to help and use Voyager for the mission instead. (VOY: "The Omega Directive")
