The Temple of Madness is a floating temple located in Terra Domina. It used to belong to Unagami and his army.
History
Prior to Prime Empire
At some point in time, the Temple of Madness was featured in an old commercial of Prime Empire to advertise the game.[1]

To stop the other ninja from arguing about the League of Jay, Nya mentioned the Temple of Madness by saying that they need the purple Key-Tana to go there.

When Lloyd, Jay, and Nya entered Terra Domina, they saw the Temple of Madness.
The Temple of Madness
Nya and Jay are able to get through the pathway to the Temple of Madness, which is a conveyor belt with rotating shurikens moving in a certain pattern.
They then head inside Sushimi's sushi restaurant where they are attacked by a Hostess, monster sushi, Sushimi's sushi chefs, and Sushimi himself.
After completing the restaurant, Jay and Nya come across the fire wall and inserts the three Key-Tanas. The two hug after going through so much, but in the background, an injured Sushimi is seen. He throws his malice at Jay, but Nya pushes him to the side causing Nya to take the hit. Jay laments over the loss, but is forced to move on.
Jay heads inside the throne room and meets Unagami. Jay claims Unagami is Milton Dyer, but Unagami corrects him by revealing that Milton Dyer is his creator. Unagami then uses Nya's energy cube to complete the Manifestation Gate. As Unagami is about to enter Ninjago, Jay pulls Unagami down believing there was a reason Dyer shut down the game. However, Unagami transform into the Empire Dragon and blazes through the portal.
Meanwhile, the Red Visors begin crossing worlds. However, some of them attack Jay. Realizing he's overwhelmed, he summons the Blue Dragon and heads into Ninjago.

Trivia
- The top of the temple's LEGO model seems to resemble a leaderboard, as it shows Unagami in first, Will in second, Jay in third, and Manovi in fourth. Manovi is the former design lead for Ninjago, but it's unknown who Will is.
- Although there is the Manifestation Gate in the show, it does not appear in the set.
