I hit this again attempting to upgrade to MediaWiki 1.31.12.

It seems similar behavior was also questioned in T212702: Special:RecentChangesLinked returns 404, which was closed as invalid with the recommendation being:

For anyone wanting to change this behaviour, I recommend going back to the original ticket and evaluating if it is still an issue and whether is can be solved differently.

Sorry for failing to follow up. I was able to get around/through this somehow, possibly with advice given in this ticket, but I don't recall how.

If you're using utf8mb4 you really should adjust your configuration to enable innodb_large_prefix (which may require adjusting some other settings as well). That seems to have become the default in MySQL 5.7 and MariaDB 10.2.2.

Then the edit summary should be modified to clarify that: It says that it "rescued" 2 sources when it fact it did not.

I see this more often when I'm away from home... Is it possible that the app is doing some sort of location/network checking at the same time? Or perhaps has not verified that it has a working Internet connection?

I can confirm that I use the beta Wikipedia Android app on both of my Android devices. When I visit Wikipedia on the web, I'm greeted with a new message of multiple failed attempted logins. (I also get a corresponding e-mail each time.)
