The Spiral Chamber was an item of Time Lord design and was present on Gallifrey.
The machine was ancient and acted as a portal into the Spiral at the nexus of the Time Vortex. Rummas stole it from Gallifrey and took it to Carsus. The Sixth Doctor used the Chamber to trap Monica Lamprey and destroy her. (PROSE: Spiral Scratch)
